Cosentyx contains the active drug secukinumab. Humira contains the active drug adalimumab. Both medications are monoclonal antibodies. These are drugs made from immune system cells. They act on specific proteins in your body.

Cosentyx belongs to a class of medications called interleukin-17A blockers. Humira belongs to a class of medications called tumor necrosis factor blockers.

Cosentyx and Humira have been approved by the Food and Drug Administration to treat the following conditions.

  • moderate to severe plaque psoriasis in adults who may need or systemic therapy

    Enbrel or Humira is usually taken together with other medications for the condition being treated. For example, an adult with moderate rheumatoid arthritis may also take methotrexate, folic acid, corticosteroids, and/or nonsteroidal anti-inflammatory drugs , in addition to the TNF blocker.

    Reasons To Change Your Tnf Blocker

    People may need to switch to a different TNF blocker for one or more reasons.

    Good reasons for considering a change include:

    • Unsatisfactory response to treatment
    • Diminished effectiveness over time
    • Undesirable side effects of treatment

    Clear expectations for treatment are important to making the decision to change medications.     Defining Treatment Failure In Psoriasis

    A generally accepted consensus definition of treatment success or failure in psoriasis has been lacking, but it would be an essential tool in patient care that would aid in treatment decision making.25 A European consensus statement acknowledged that while the ultimate goal of psoriasis treatment is complete clearance of cutaneous lesions, a definition of treatment success or failure needs to be based upon the results achievable with current therapies.25 The consensus recommended defining treatment success in patients undergoing induction and maintenance therapy as a reduction in the Psoriasis Area and Severity Index of at least 75 percent compared to disease severity at the time of treatment initiation. An intermediate response was considered to be achievement of PASI 50 and < 75, with a Dermatology Life Quality Index score of 5. The consensus statement recommended treatment modification if an intermediate response is not achieved at the end of the induction phase or during bimonthly follow-ups for safety monitoring during the maintenance phase. However, the utility and clinical relevance of this guidance is limited because PASI measurements are rarely performed outside of research trials.

    Considerations For Switching Therapy

    Despite the fact that treatment optimization is important to maximize improvement in psoriasis and that current guidelines provide information on altering treatment regimens when patients fail to achieve desired treatment goals, decisionmaking criteria for switching therapy are not well defined, and there are limited data on how to transition from one treatment to another in routine clinical practice 18. Part of the reason why this guidance is only now becoming available 18 is that the practice of switching has only recently evolved. In the past, the rationale for switching treatments was often related to safety concerns and involved rotating between conventional systemic agents with different targetorgan toxicities in order to reduce cumulative exposure 25.

    For example, an international consensus report recommended that cyclosporine should only be used intermittently for 36 months, and the package insert cautions against continuous treatment longer than 1 year 18, 26. The riskbenefit profile of cyclosporine must be carefully considered, particularly in older patients, as longterm use can significantly increase risks of renal toxicity, hypertension, and skin cancer. Skin cancer risk is especially high in patients previously treated with psoralen plus ultraviolet A therefore, switching from PUVA to cyclosporine is generally not recommended 12. When discontinuing cyclosporine treatment, it is important to note that abrupt cessation can cause psoriasis flares 12.

    Switching patients with moderate-to-severe plaque psoriasis from one biologic to another is a multifactorial decision and may depend on safety, efficacy or dosing intervals. Ron Vender, M.D., offers insight into how to switch appropriately.

    With many biologics now available for patients with moderate-to-severe plaque psoriasis, physicians may find themselves transitioning among them to optimize treatment and improve quality of life, according to Ron Vender, M.D., associate clinical professor of dermatology, McMaster University, and founder and director, Dermatrials Research, Inc, Hamilton, Ontario, Canada.

    Switching among agents is one strategy to consider when patients fail to achieve an adequate response to biologic therapy, experience secondary failure with loss of efficacy over time, or develop an adverse reaction, he says. Dose or dosing interval adjustments may also be considered. In addition, there are also reports that repeating the induction phase regimen can help patients who are secondary failures.

    The Importance Of Treatment Goals

    As discussed above, there are different thresholds that can be used to measure the success of psoriasis treatments . However, these measures of disease severity are not routinely used in clinical practice. Treatment goals are usually subjectively assessed in this setting by a 10point patientassessed or physicianassessed visual analog scale or a modified PGA/Investigator’s Global Assessment 22 scale and progress is discussed with the patient. Patients and physicians often have very different expectations of the extent of disease control that will be achieved with treatment therefore, communication between patients and practitioners is essential to set agreedon treatment goals 12, 23. Treatment goals should be tailored based on disease severity and the degree of improvement that is possible. However, individual treatment goals can vary considerably, even between patients with similar disease severity. Treatment goals should be clearly discussed with the patient when initiating care in order to align patient and physician expectations. In addition to skin signs and symptoms, important factors to consider when setting treatment goals are the impact that psoriasis has on the patient’s quality of life and the impact of comorbidities on the patient’s overall health 12.

    • No efficacy difference was found between certolizumab pegol and adalimumab in combination with MTX at both 12-weeks and 2-years in MTX-inadequate responders with RA

    Patients were randomized to receive either certolizumab pegol plus MTX or adalimumab plus MTX . After 12 weeks, patients with an adequate response to treatment stayed on the same agent, while non-responders were switched to the other treatment for the duration of the study, which totaled 104 weeks. The switch was undertaken without a drug washout period.

    At 12 weeks, 69% of patients who received certolizumab pegol plus methotrexate achieved a 20% improvement response as defined by American College of Rheumatology criteria , compared to 71% of those who received adalimumab plus methotrexate.

    At two years, 35% of patients in the certolizumab pegol arm achieved a state of low disease activity vs. 33% of those in the adilumumab arm.

    Of patients classified as non-responders at week 12, 14% were in the certolizumab pegol arm and 13% were in the adalimumab arm. Fifty-eight percent of non-responders switched to certolizumab pegol and 62% of switched to adalimumab achieved low disease activity or reduction in Disease Activity Score 28-erythrocyte sedimentation rate 1.2 12 weeks following the treatment change.

    These study results suggest that switching to a second TNF inhibitor after primary TNF inhibitor failure in methotrexate inadequate responders is safe and effective, even without a drug washout period.
