Is There Anyone Who Should Avoid Taking Norethisterone
“Norethisterone is safe for the majority of women,” Wilson says. “It should not be used for women who have suffered with liver tumours, breast cancer, a history of jaundice during pregnancy, and severe vascular disease.”
You should avoid taking the medication if you are pregnant or think you could be, if you are breastfeeding, if you have abnormal vaginal bleeding that hasnt been investigated by a doctor, or if you have a known, suspected or past history of genital or breast cancer .
Those who have recently had angina, a heart attack or a stroke caused by a blood clot should also avoid taking norethisterone.
Dr Lucinda Farmer, spokesperson for the Faculty of Sexual and Reproductive Healthcare , says norethisterone has been shown to be partly metabolised to become the oestrogen hormone ethinylestradiol.
“Women being prescribed 5 mg of norethisterone to postpone menstruation could be exposed to 20-60 mcg of ethinylestradiol a day, which is equivalent to the amount found in two combined contraceptive pills,” she says. “Care should be taken when prescribing these doses of norethisterone in women with contra-indications to the combined pill, especially women presenting with risk factors for venous thromboembolism.”

Does Norethisterone Stop Bleeding When Taken For Menorrhagia
Gail Rogan, 44, first went to see her GP about heavy periods in February 2018. Her periods were getting progressively longer and heavier, and she decided enough was enough. Night-time pads became saturated within 1-2 hours and I had to call in sick to work for a week as I was unable to do anything, she says, adding that she slept most of the time and only woke to change and drink water.
Originally her GP prescribed her mefenamic acid, an anti-inflammatory drug used to treat pain, and blood tests found she was low in iron. Things continued and each month her period got longer. Her GP changed her prescription to a mini-pill, but this had little effect, and eventually she was put on the waiting list to see a gynaecologist. By the end of August my period was lasting 25 days, she says. I was physically and emotionally drained and just completely broke down in my GPs office. She recommended that I take some time off work and Ive been out for two months now.
She saw a gynaecologist who recommended she take norethisterone for three weeks each month with one week off, taking tranexamic acid on the week off. I started this at the end of September, she says. The gynaecologist was confident that I would not bleed at all during the three weeks on the norethisterone. Gail decided to go ahead with norethisterone because is only a temporary solution until she gets a Mirena coil fitted when she has a hysteroscopy.
Side Effects And Risks
A risk of using a hormonal contraceptive method too soon after starting menstruation is the closing of the epiphyses of the bones, which can result in stunted growth. People who start taking hormonal contraceptives shortly after their first period may stop growing in height.
Depending on the type of hormonal contraceptive, some people may experience breakthrough bleeding. Breakthrough bleeding occurs more often when people first start using a hormonal contraceptive. Usually, this side effect decreases over time.
People who choose to use hormonal injections need to see a healthcare professional every 3 months for their doses. Missing an appointment can put a woman at risk of pregnancy if she is also using it as a contraceptive.
Researchers have shown that females who use medroxyprogesterone injections have lower average bone mineral density. This effect has no link with increased fracture risk, though.
People with obesity or appetite control difficulties who receive medroxyprogesterone injections may gain 1020 kilograms of weight. However, people with a moderate body weight tend not to gain the extra weight.

The combined contraceptive pill can stop your period over a longer period of time but you can get light bleeding if you dont take a break every month. So if you want to guarantee no period bleeding, youll need to have a 7-day break every month when you might sometimes have period-like bleeding.

How Does Norethisterone Work
Norethisterone is a man-made version of the female sex hormone progesterone. It works by mimicking the effects of your natural progesterone.
Your progesterone levels naturally fluctuate throughout your monthly menstrual cycle. As progesterone levels increase your womb lining flourishes, ready to receive a fertilised egg. If you don’t get pregnant the levels fall again, causing your womb lining to be shed as a period. If you do get pregnant your progesterone levels remain high to help maintain a healthy womb lining for the ongoing pregnancy.

Can Eating Carotene Containing Foods Affect The Menstrual Cycle
It is said that eating beta-carotene, in orange foods such as papaya, carrots, pumpkins, and pomegranate, raises the level of oestrogen causing an early period. Bringing a period forward can avoid having a bleed at a particular time. There is evidence that beta-carotene varies with the menstrual cycle, and that there is a link between beta-carotene and oestrogen levels. However the exact mechanisms remain unclear and eating the above foods is not proved to have the desired effect.
Taking 2 Packets Of The Combined Pill Back
If you take a combined contraceptive pill, you can delay your period by taking 2 packets back-to-back.
How you do this will depend on which pill you take.
Examples are:
- monophasic 21-day pills, such as Microgynon and Cilest you take a combined pill for 21 days, followed by 7 days without pills, when you have a bleed . To delay your period, start a new packet of pills straight after you finish the last pill and miss out the 7-day break.
- everyday pills, such as Microgynon ED and Lorynon ED you take a combined pill every day. The first 21 pills are active pills and the next 7 pills are inactive or dummy pills, when you have your period. To delay your period, miss out and throw away the dummy pills, and start the active pills in a new packet straight away.
- phasic 21-day pills, such as Binovium, Qlaira and Logynon the mix of hormones in each pill is different, depending on which phase you’re in. You need to take these pills in the correct order to have effective contraception. Ask your pharmacist, community contraception clinic or GP for more information.
Taking your contraceptive pills in the ways described above will not affect how they work as contraceptives.

Why Do I Get Monthly Periods Whilst Taking The Pill
The pill controls the female hormones. One of the effects is stopping the thickening of the lining of the uterus , which would normally occur in preparation for implantation of a fertilised egg. Thus periods are not needed. The doctors who first developed the pill in 1958, decided that women would want monthly bleeds, as it would feel natural. Every three weeks the break between packs or placebo pills result in decreasing hormone levels and a withdrawal bleed. As the lining is very thin, this bleeding is light. It is safe to miss the pill free interval and your period as above. Extended-cycle birth control pills, which are taken for 12 weeks with a withdrawal bleed in the 13th, are available in the US, but not in the UK. In the UK the same effect can be achieved by running packs of pills together. The Faculty of Sexual and Reproductive Health in the UK now recommends several routines of extended use of combined pills which reduce the bleed frequency.
Case Control Study Consisting Of Orthodox Jewish Brides
Treatment with 5mg norethindrone acetate three times daily was begun on or before day 12 of the menstrual cycle before the wedding. Norethindrone was begun at least 3weeks before the wedding day and continued until the marriage was consummated. Treatment duration never exceeded 35days. Patients who missed a dose were told to take the medication as soon as they remembered to. The mean age was 22 and nulligravida. A control group consisted of 4 married women from the same ethnic background and age group who desired contraception and were started on OCPs the day of their visit and instructed to use a back-up method for 7days. A questionnaire at follow-up determined presence of spotting and side-effects. Researchers were blinded to the treatment group.

Bad Experiences With Norethisterone
Mollie Davies , 21, was prescribed norethisterone for menorrhagia when she was about 14. She was already on mefenamic acid and various other pills, but was in so much pain she said she would have taken everything. Her doctor described it as a magical tablet that would stop her periods when she was on holiday or swimming, so while she didnt take it continuously, she used it whenever she wasnt in a position to be able to cope with her extremely heavy periods.
When she was 17 she took it over several days for a holiday. I did everything as I was told and woke up one morning in excruciating pain. I was screaming, on all fours. I panicked and thought something was seriously wrong. After an hour or so of short pains on and off, suddenly these sped up and lasted a couple of minutes. Then a palm sized ball of flesh covered in blood dropped to the floor, she says. I instantly felt better, but bled for the next few days. It pretty much ruined my holiday.

While Mollies story is probably an extreme example of a bad experience, it pays to be aware of the pros and cons when you are considering norethisterone. Follow your doctors advice, and make yourself aware of the numerous other treatment options there are out there for menorrhagia.
